- Send an application (there is no need to motivate your application. Merely state that you are applying for an authorisation for veterinary surgeon. Do also write the physical address to where you want to receive the hard copy of your authorisation)
- Attach the following documents to your application:
- Proof of identity and nationality (copy of your valid EU identity card or passport)
- Your diploma (copy of your diploma in the original language)
- Letter of good standing (The letter of good standing must be issued from the competent authority in the country where you have obtained your diploma. The letter of good standing must be valid and must be issued not more than 3 months before we receive it)
- The veterinary oath (fill out the oath and sign it: download the veterinary oath)
- Your CPR Number (only if you live in Denmark. You can find the number on your Health insurance card)
